PLEASE READ BEFORE SCHEDULING YOUR APPOINTMENT!

Please review the following steps before you schedule an appointment on the website. 

Getting our families vaccinated plays a crucial role in keeping our airmen mission capable and is yet another step toward bringing an end to this global pandemic. The California Military Department and the 146th Airlift Wing are excited to announce we're extending COVID-19 vaccination appointments to all military dependents registered within DEERS effective immediately. We are honored to be able to provide this additional layer of protection for our military families as the number of vaccines available increases throughout the State of California and across the nation. 

Our goal is to reduce the time dependents will have to wait to receive the vaccine. If you have any questions regarding scheduling - please see our frequently asked questions section. Because of the high demand for scheduling and increased call volume, we ask that you DO NOT call our medical group's direct line to schedule an appointment. Please use this website to email the Medical Group's org box to schedule your appointment, as this will be the fastest method of communication. Thank you for your patience and understanding as we work together to help end the COVID-19 pandemic.

SCHEDULING EXPECTATIONS

PLEASE WEAR AN APPROPRIATE TOP TO ALLOW EASY ACCESS TO YOUR SHOULDER

FACE MASKS MUST BE WORN AT ALL TIMES.

  • By scheduling an appointment at our Medical Group facility, you agree to schedule your second appointment after receiving your first dose during your first appointment. You will be contacted via the contact number and email you provided by our scheduling team before your requested meeting to confirm the day and time of your COVID-19 vaccine.

  • It is IMPERATIVE that you report on time the day of your appointment as we will have multiple groups scheduled during the day. If you are late for your appointment, we cannot guarantee you will be able to receive your COVID-19 vaccine that day. Please email the medical group with the subject line RESCHEDULE APPOINTMENT in the comment box below as soon as you suspect you cannot make your appointment.

  • Upon arrival, individuals who request to receive the vaccine will be required to fill out a Form DHA 207 and will then be screened by a medical professional.

  • For safety, social distancing precautions, and in the event you have an adverse reaction to the vaccine, we ask that you DO NOT bring minors (under 18) with you to your appointment, as our facility is limited in space, nor do we have the ability to provide care for them. If you do bring an accompanying minor, we request that you get an additional adult to supervise them while remaining in your vehicle during your appointment. 

  • There is a mandatory 15-minute waiting/observation period after receiving the shot and before departure.

3. Take screen shot including the "Family Members" section on left side

4. Print screen shot and bring document with you to vaccination appointment

STEP TWO: CONFIRM HEALTH HISTORY

Possible delays in scheduling:

There are a few things that could possibly delay your appointment due to medical treatment or medical history; please review the following items that would preclude you from receiving the vaccine at the moment. If any of the following pertain to you, please annotate this in your request form and let our Medical Group know before scheduling your appointment.

• Receiving another vaccine within 14 days of the COVID Vaccine scheduled administration date

• History of Anaphylaxis to other vaccines or COVID vaccine components

• Being currently in isolation or quarantine for COVID exposure or COVID positive

• Being currently ill, such as with a fever or other symptoms of COVID or in the past 72 hours, CONSULT YOUR MEDICAL PROVIDER Check with your medical provider to determine if you should receive the vaccine before scheduling an appointment.

FREQUENTLY ASKED QUESTIONS

WHAT SHOULD YOU MENTION TO YOUR VACCINATION PROVIDER BEFORE YOU GET THE MODERNA COVID-19 VACCINE?

Tell your vaccination provider about all of your medical conditions, including if you:

• have any allergies

• have a fever

• have a bleeding disorder or are on a blood thinner

• are immunocompromised or are on a medicine that affects your immune system

• are pregnant or plan to become pregnant

• are breastfeeding

• have received another COVID-19 vaccine

WHO SHOULD GET THE MODERNA COVID-19 VACCINE?

FDA has authorized the emergency use of the Moderna COVID-19 Vaccine in individuals 18 years of age and older.

WHO SHOULD NOT GET THE MODERNA COVID-19 VACCINE?

You should not get the Moderna COVID-19 Vaccine if you:

• had a severe allergic reaction after a previous dose of this vaccine

• had a severe allergic reaction to any ingredient of this vaccine

WHAT ARE THE INGREDIENTS IN THE MODERNA COVID-19 VACCINE?

The Moderna COVID-19 Vaccine contains the following ingredients: messenger ribonucleic acid (mRNA), lipids (SM-102, polyethylene glycol [PEG] 2000 dimyristoyl glycerol [DMG], cholesterol, and 1,2-distearoyl-sn-glycero-3-phosphocholine [DSPC]), tromethamine, tromethamine hydrochloride, acetic acid, sodium acetate, and sucrose.

HOW IS THE MODERNA COVID-19 VACCINE GIVEN?

The Moderna COVID-19 Vaccine will be given to you as an injection into the muscle. The Moderna COVID-19 Vaccine vaccination series is 2 doses given 1 month apart. If you receive one dose of the Moderna COVID-19 Vaccine, you should receive a second dose of the same vaccine 1 month later to complete the vaccination series.

WHAT ARE THE BENEFITS OF THE MODERNA COVID-19 VACCINE?

In an ongoing clinical trial, the Moderna COVID-19 Vaccine has been shown to prevent COVID-19 following 2 doses given 1 month apart. The duration of protection against COVID-19 is currently unknown.

WHAT ARE THE RISKS OF THE MODERNA COVID-19 VACCINE?

Side effects that have been reported with the Moderna COVID-19 Vaccine include:

• Injection site reactions: pain, tenderness and swelling of the lymph nodes in the same arm of the injection, swelling (hardness), and redness

• General side effects: fatigue, headache, muscle pain, joint pain, chills, nausea and vomiting, and fever. There is a remote chance that the Moderna COVID-19 Vaccine could cause a severe allergic reaction. A severe allergic reaction would usually occur within a few minutes to one hour after getting a dose of the Moderna COVID-19 Vaccine. 

For this reason, your vaccination provider may ask you to stay at the place where you received your vaccine for monitoring after vaccination. Signs of a severe allergic reaction can include:

• Difficulty breathing

• Swelling of your face and throat

• A fast heartbeat

• A bad rash all over your body

• Dizziness and weakness

These may not be all the possible side effects of the Moderna COVID-19 Vaccine. Serious and unexpected side effects may occur. The Moderna COVID-19 Vaccine is still being studied in clinical trials.

WHAT SHOULD I DO ABOUT SIDE EFFECTS?

If you experience a severe allergic reaction, call 9-1-1, or go to the nearest hospital. Call the vaccination provider or your healthcare provider if you have any side effects that bother you or do not go away. Report vaccine side effects to FDA/CDC Vaccine Adverse Event Reporting System (VAERS). The VAERS toll-free number is 1-800-822-7967 or report online to https://vaers.hhs.gov/reportevent.html. Please include “Moderna COVID-19 Vaccine EUA” in the first line of box #18 of the report form. In addition, you can report side effects to ModernaTX, Inc. at 1-866-MODERNA (1-866-663-3762).

You may also be given an option to enroll in v-safe. V-safe is a new voluntary smartphone-based tool that uses text messaging and web surveys to check in with people who have been vaccinated to identify potential side effects after COVID-19 vaccination. V-safe asks questions that help CDC monitor the safety of COVID-19 vaccines. V-safe also provides second-dose reminders if needed and live telephone follow-up by CDC if participants report a significant health impact following COVID-19 vaccination.
